Fill
in a housing needs register application form which you can get from
Westlea Housing Association or the distrcit council offices in Monkton
Park, Chippenham. Please fill in the form as fully as possible. If you
are going to have a joint tenancy with your partner when you are
rehoused, please tick the box showing that you want a joint application.
If you have any difficulties with the form, please call in to the
office, or phone for help. Take the filled-in form to the Westlea
Housing Association office, or send it to the address given at the end
of the application form. When
you send a new application, you will be sent a letter to tell you that
North Wiltshire District Council has received your application. This
will tell you your registration number and, if appropriate, the number
of points we have awarded you. if you do not receive this letter within
four weeks of returning the application, please call into the housing
association office or phone the Housing Helpline on 01249 466150. Pre-Tenancy
Services staff aim to visit you at home before offering accommodation.
If you ask for a personal visit, staff will be happy to oblige, either
in your own home or at another address if this is necessary. If
you or any member of your household have any medical problems, or
disabilities, you should show this on the application form. You will
then be sent a medical assessment form. Westlea may also contact your
GP, a specialist or social services if they are giving you support and
will also arrange for you to be visited at your present home by a
Housing Officer. Westlea can award up to 15 medical points, but will
only give these if the medical condition or disability is to do with
your present housing situation, and could be helped by rehousing. |
